    Appointments
    You will require a referral letter from your GP or specialist. Please bring copies of any x-rays/imaging, medicare card and private insurance details. You can also save time by filling out the new patient form below.

    Fees
    Accounts are required to be settled on the day of your consultation. A proportion of the fee is rebatable from Medicare. We accept cash, cheque, EFTPOS, Visa and Mastercard. WorkSafe and TAC patients will only be seen if standard consultation fees are settled upfront on the day of consultation. Please note that this requires a co-payment.

    Public patients
    If you do not have private health insurance, please ask your local doctor or specialist to refer you to the Austin Hospital where Mr Chin has a public appointment.
